What screening tools are used?

The Ages and Stages Questionnaire (ASQ-3) measures your child’s development in the areas of Communication, Gross Motor, Fine Motor, Problem Solving and Personal-Social skills.

The ASQ:Social-Emotional (ASQ:SE-2) focuses on your child’s Social-Emotional development – including sleep habits, toilet habits, mealtimes and behaviors.

The ASQ-3 and ASQ SE-2 take about 30-40 minutes to complete.

What happens after the screening?

An early childhood professional will send you the results of your child’s ASQ-3 within 5 business days and help you make a plan of action for your child’s continued development.

What will I need to complete a screening?

You will need some household items and basic toys to complete the screening activities.

2 through 12 Month Questionnaires: clear plastic baby bottle, 2 small blocks or toys, mirror, Cheerios or other small food, cup or bowl, shoelace or yarn, small ball, crayons, paper, book with pictures, large ball, cracker or cookie, stuffed toy or doll, spoon and fork

14 through 24 Month Questionnaires: clear plastic baby bottle, 6 small blocks, small ball, crayons, paper, Cheerios or other small food, 4 beads or noodles and string or yarn, book with pictures, large spoon or scoop

27 through 36 Month Questionnaires: 7 small blocks, crayons, paper, 4 beads or noodles and string or yarn, child safe scissors, zipper, book with pictures, small ball, large ball

42 through 60 Month Questionnaires: small ball, 4 small blocks, crayons and paper, puzzle with 5-7 interlocking pieces, child safe scissors, zipper, book with pictures, something with a button
